There will be a $35.00 service charge on all returned checks or the maximum amount permitted by law.

Please review the notice you received from us carefully. It is our practice to report most unpaid valid accounts to credit bureaus for listing in credit files 45 days from the date of our first collection notice. Payments made after 45 days are reported to the credit bureaus so they can update their records.

Primary and secondary medical insurance information provided by you to us via this website will be sent to our client to review. By providing this information you are authorizing them to submit an insurance claim on your behalf.

Since your account is now in collection, please note that providing insurance information will not prevent the account from being sent to the credit reporting agencies for listing in your credit file unless our client asks us to halt credit reporting.

This website provides you with the opportunity to set up a payment plan for your account. Please note that setting up a payment plan will not prevent the account from being sent to the credit reporting agencies to be listed in your credit file unless the account is paid in full within 35 days of date of the first collection notice we sent you. Please review the notice we sent you carefully. If you have any questions, need to change your payment plan or need to know the date the account will be listed on your credit file please call us toll free at 800-325-4013.

When you provide us with a wireless telephone number or land line number you are giving us your express consent to call that number.

Indemnity

You agree to defend, indemnify, and hold harmless DataMax and its subsidiaries, and their employees, contractors, officers, and directors from and against all liabilities, claims, and expenses, including reasonable attorney's fees, that arise from your use or misuse of this Site.

Changes in Terms of Use

DataMax reserves the right to update or revise these Terms of Use at any time. Please check the Terms and Conditions periodically for changes. Your continued use of this site following the posting of any changes to the Terms of Use constitutes acceptance of those changes.

Entire Agreement
